 With reference to existing MNMC:19 / 90 Nurse and Midwife Law and Procedures

“Policies for Applying Registration& Licence; Temporary Registration& Licence;  Re-licence; Duplicate of Certificate, licence and Registration; and current and active registration  for Nurses and Midwives”

 ( For local nurses and Midwives )

 

LICENSE AND REGISTRATION

Session (A) Policy and criteria for applying “ License and Registration”

This policy and regulatory criteria will be applied for those nurse and midwife graduates working in their appointed postings. The following necessary documents / procedures are as follow:-

  1. Fill up application form completely
  2. Nursing Midwifery Degree, or  Diploma or Certificate ( True Copy and Photocopy)
  3. National Identity Card – Photo copy
  4. Photocopy and true copy of Bank slip document currently in Bank No 4, Yangon, Account No.CM068738
  5. Recommendation  of job performance competency from respective supervisor
  6. 6.      Copy of recommendation on job competency for those who are relief from job or medical pension
  7. Attach a copy of office document for “dismiss, retirement and job relief”
  8. Official original document of Office Seal and Personal Seal must be printed in all departmental recommendations
  9. The license and registration will be issued to representative person whose national identity, owners’ National  identity and  letter of authority must be accompanied

Session ( B ) Temporary license and Registration

Requirements for applying “Temporary License and Registration”.

This policy and regulation criteria are required for those Nurse or Midwife graduates waiting for their postings.

  1. Fill up an application form completely
  2. National Identity Card – Photo copy
  3. Photocopy and true copy of Bank slip document currently in Bank no 4 , Yangon, Account No.CM068738
  4. Recommendation for completion of Nursing Midwifery Education Programme (True copy & Photo Copy)
  5. Passport photo ( 5 ) with full uniform
  6. Office seals originals are required in all recommendation certificates
  7. The Temporary license will be issued to representative person whose national identity, owners’ National  identity and letter of authority must be accompanied

 

Session ( C ) Policy and criteria for applying “Re-Licence”

This policy and regulatory criteria will be applied for those nurse and midwife graduates working in their appointed postings.

  1. Fill up an application form completely
  2. National Identity Card – Photo copy
  3. Photocopy and true copy of Bank slip document currently in Bank no 4 , Yangon , account No.CM068738
  4. Nursing Midwifery Degree or Diploma or Certificate , and registration ( Copy )
  5. Document for recommending  job performance competency and evidence of working as in-service
  6. Copy of documents for “ Retirement, Dismissed, removed  and relief from job”( for those concerned personal only )
  7. Document for recommending job performance competency by Nursing Officers from States and Regional Department of Health, if he or she is retired with medical ground or relieved from job or dismissed.
  8. Official original document of office Seal and personal seal must be printed in all recommendations
  9. The re-license  will be issued to representative person whose national identity, owners’ National  identity and letter of authority must be accompanied

 

 

Session ( D ) Issuing duplicate of Certificate and Registration 

Policy and criteria for applying to issue duplicate of “Certificate and Registration”

This policy and regulatory criteria will be applied for those nurse and midwife graduates whose Registration and Certificate are lost /damaged by legal means  or due to Natural Disasters

  1. Fill up an application form completely
  2. National Identity Card – Photo copy
  3. Photocopy and true copy of Bank slip document currently in Bank 4, Yangon, , Account No.CM068738
  4. Passport photo ( 5 ) with full Uniform / or full uniform with student cap for certificate
  5. License  must be submitted
  6. True copy of recommendation  endorsed by nearest police department
  7. Submit destroyed registration or certificate, if possible
  8. 7.      If you are Government Service, submit recommendation from your head of department
  9. 8.      Document of recommending job performance competency by nursing officer( Format to be used)
  10. 9.      Submit recommendation for actual damage or lost by your nursing officer from State or Regional Health Department if you are retired or relieved from job or dismissed.
  11. 10.  Submit self commitment of actual lost
  12. Official original document of office Seal and personal seal must be printed in all recommendations
  13. The true copy of certificate and registration will be issued to representative person whose national identity, owners’ National  identity and letter of authority must be accompanied

Note:- For midwifery certificate MNMC will be considered before jan.2002, and for  nursing certificate MNMC will be considered before 2001

 

Session ( E ) Duplicate of License

Policy and criteria for applying to issue copy of “Duplicate of  Licence”

This policy and regulatory criteria will be applied for those nurses and midwife graduates whose License is lost with legal means or due to Natural Disasters

  1. Fill application form completely
  2. National Identity Card – Photo copy
  3. Photocopy and true copy of Bank slip document currently in Bank (4), Yangon, , Account No.CM068738
  4. Copy of nursing or midwifery Degree or Diploma or Certificate and registration
  5. 4.      Passport photo ( 5 ) Copies with full uniform
  6. 5.      Original testimony endorsed by nearest police department recommending that it is actual lost
  7. Document for recommending job performance competency by your immediate nursing officer
  8. Self commitment of actual lost, or damage  must be submitted
  9. Submit recommendation for actual damage or lost by nursing officer from State and Regional Health Department if you are retired or relieved from the job or dismissed
  10. Official original document of office Seal and personal seal must be printed in all recommendations
  11. The duplicate of license will be issued to representative person whose national identity, owners’ National  identity and letter of authority must be accompanied

 

 

 

Session ( F ) Current Registration & Active Registration

Policy and criteria for applying to issue “Current Registration & Active Registration”

This policy and regulatory criteria will be as follows:-

  1. Fill up an application form completely
  2. National Identity Card – Photo copy
  3. Photocopy and true copy of Bank slip document currently in Bank (4), Yangon, Account No.CM068738
  4. 4.      Passport color photo ( 4 ) with full uniform
  5. Copy of nursing or midwifery Degree or Diploma or Certificate and registration
  6. 6.      Original paper of recommendation by police that you are free from crimes
  7. Original document and copy of relief from job
  8. Original document and copy of refunding ( Bond )
  9. Up to date license or re-license
  10. The Current Registration & Active Registration will be issued to representative person whose national identity, owners’ national  identity and letter of authority must be accompanied

 

 

NEW REGESTRATION AND LICENCE

Session (A)  Policy and criteria for applying “REGISTRATION FOR NEW GRADUATE”

This policy and regulatory criteria will be applied for those nurse and midwife who are newly graduated .

  1. Fill up an application form completely
  2. Nursing Midwifery Degree, or  Diploma or Certificate ( True Copy and photocopy )
  3. National Identity Card – Photo copy
  4. Photo (5) with full uniform
  5. Photocopy and true copy of Bank slip document current in Bank No 4, Yangon, Account No.CM068738

 

Session ( B )  Policy and criteria for applying “REGISTRATION FOR WORKING IN THE POST”

This policy and regulatory criteria will be applied for those graduate nurse and midwives who are working in their appointed postings ( Under Government Services).

  1. Fill application form completely
  2. Nursing Midwifery Degree, or  Diploma or Certificate ( True Copy and photo copy )
  3. National Identity Card – Photo copy
  4. Photocopy and true copy of Banking slip document from Bank No 4, Yangon, Account No.CM068738
  5. Recommendation  of job performance competency from respective supervisor

 

Session ( C ) Policy and criteria for applying “REGISTRATION WHO ARE NOT APPOINTED IN THE POSTS”

This policy and regulatory criteria will be applied for those graduate nurses and midwives who are not appointed in the posts( Usually Government Posts).

  1. Fill application form completely
  2. Nursing Midwifery Degree, or  Diploma or Certificate (True  Copyand photo copy )
  3. National Identity Card – Photo copy
  4. Photocopy and true copy of Bank slip document currently in Bank No 4, Yangon, Account No.CM068738
  5. 4.      Pass competency examination conducted by MNMC

 

Session (D ) Policy and criteria for applying “PRACTICING LICENCE  WHO ARE REGISTERED AND APPOINTED IN THE POST”

This policy and regulatory criteria will be applied for those graduate nurses and midwife who are REGESTERED and appointed in the Government posts.

  1. Fill application form completely
  2. Nursing Midwifery Degree, or  Diploma or Certificate ( True copy and photo copy )
  3. National Identity Card – Photo copy
  4. Passport Photo ( 5) with full uniform
  5. Photocopy and true copy of Bank slip document currently in  Bank No 4, Yangon, No.CM068738
  6. True copy and photo copy of REGISTRATION
  7. 6.      Recommendation  from State and Regional Health Department

 

Session ( E )  Policy and criteria for applying “TEMPORARY PRACTICING LICENCE WHO ARE REGISTERED AND NOT APPOINTED IN THE POST  ”

This policy and regulatory criteria will be applied for those graduate nurses and midwives who are REGESTERED and not appointed in the government posts.

  1. Fill application form completely
  2. Passport Photo ( 5) with full uniform
  3. Nursing Midwifery Degree, or  Diploma or Certificate ( Copy )
  4. National Identity Card – Photo copy
  5. Photocopy and true copy of Bank slip document current in Bank No 4, Yangon, Account No.CM068738
  6. True copy and photo copy of REGISTRATION
  7. Duration Licensure last “ One Year “
  8. 7.      If not posting is not available extend another year as temporary license
  9. 8.      If temporary license holder join posting, can apply license with document of appointment and working evidence.

 

 

Session ( F )  Policy and criteria for applying “LICENSING WHO ARE REGISTERED AND LICENCE HOLDER AND DISMISSED FROM JOB ”

This policy and regulatory criteria will be applied for those graduate nurses and midwfie who are REGESTERED and license holder and dismissed from the job.

  1. Fill application form completely
  2. Nursing Midwifery Degree, or  Diploma or Certificate (True Copy and photocopy )
  3. National Identity Card – Photo copy
  4. Photocopy and true copy of Bank slip document current in Bank No 4, Yangon, Account No.CM068738
  5. Photo copy of REGISTRATION
  6. True copy of LICENCE must be submitted to MNMC
  7. Self commitment of refrain from involvement in Parties and politics
  8. 7.      True and photo copy of dismissed order
